Consent forms for the flu vaccines are going home with all students this week for parents to sign. There are two signatures required, one for the seasonal flu vaccine and one for the H1N1 vaccine.
The flu clinic at Academics Plus Charter School will be held Friday October 23 from 9 am to 2 pm and again on Friday Nov. 6 from 9am to 2pm.
A consent form MUST be signed by the parent and returned to the nurse in order for you child to receive the vaccine.
There is no cost for the vaccine.
